Every family deserves a little extra care and attention when they bring a new baby home.  A new baby, lack of sleep and a healing body can feel overwhelming.  Family Connects Chicago at Saint Anthony Hospital can help. Family Connects is a free service that extends care for you, your baby and your family after you leave the hospital.

Special Care After Giving Birth

Family Connects involves an in-home or online visit with a registered nurse. We connect you to care and services in your own community that can help you thrive physically, mentally, emotionally and socially.

The free service is available to all families who deliver babies at Saint Anthony Hospital and live in Chicago.  Our Family Connects services are a partnership with the Chicago Department of Public Health.

How Family Connects Works

A Family Connects team member will reach out to see if you would like an at-home or online visit and find the best time for you and your family. 

The visit is to get the care and support you need.  Your nurse is there to provide care, answer your questions, and connect you to community resources that will ensure you and your family have everything you need to be happy and healthy. 

Family Connects is here to support every Chicago family with whatever they might need in the first weeks and months with a baby.  We can assist you with:

Help for Baby

  • Baby weight and health check
  • Safe sleep information 
  • Infant feeding and fussiness
  • Help with bathing, diapering and swaddling

Help For Birthing Parent

  • Parent health check 
  • Breastfeeding support
  • Family planning advice
  • Postpartum depression screening

  • Scheduling doctor’s appointments
  • Understanding childcare options
  • Early literacy information 
  • Community resources and connections
