Illinois Hospital Report Card Act

Saint Anthony Hospital is committed to delivering the highest quality care to all of our patients.

In accordance with the Illinois Hospital Report Card Act, patients, family members, and the general public may request nurse staffing information.

Accordingly, the following information is available to the public upon request:

  • nursing assignment rosters
  • nursing schedules for each inpatient unit
  • methodology to determine and adjust nurse staffing levels
  • information on staff training and orientation.

Currently Hospitalized Patients

If you, or a family member, are currently hospitalized, you have the right to ask for this information, as well as information about the nursing staff's credentials, including education, training and certification.

During normal business hours (Monday through Friday, 9:00 a.m. to 5:00 p.m.), please ask your nurse to contact the department manager for assistance in obtaining the information. Requests from, or on behalf of, currently hospitalized patients will be handled within two hours during normal business hours.

To obtain information outside of normal business hours, call 773.484.1000 and ask for the House Manager. Requests outside of normal business hours will be handled within four hours.

Requests from the Community or Others Not Currently Hospitalized

Requests from community members and others who are not currently hospitalized will be addressed within seven business days. Information may be requested in writing.
