Code of Conduct Attestation
Standards of Conduct and Professionalism:
Treat all persons, including patients, families, visitors, employees, trainees, students, volunteers, and healthcare professionals, with respect, courtesy, care, dignity, and a sense of fairness. Recognize and be sensitive to the needs of individuals from diverse backgrounds (including gender, race, age, disability, nationality, sexual orientation, and religion).
Communicate openly, respectfully, and directly with team members, referring providers, patients, and families to optimize health services and promote mutual trust and understanding.
Encourage support and respect the right and responsibility of all individuals to assert themselves to ensure patient safety and quality care.
Respect the privacy and confidentiality of all individuals. Adhere to all SAH policies and HIPAA regulations regarding protected health information.
Uphold the policies of SAH.
Participate in education and training required to perform job duties.
Do not disseminate false, misleading, or deceptive information regarding SAH and individuals associated with SAH.
Behavior:
Refrain from the use of profanity or similarly offensive language while at SAH.
- Not shout, raise voices, make discourteous comments, demands, or name-calling to any individual at SAH.
- Do not make discourteous comments, demands, or name-calls to any individual at SAH.
- Not criticize any individual at SAH in front of or within earshot of any other individual. Address any criticisms or concerns courteously, constructively, and privately.
- Do not physically or otherwise threaten or sexually harass any individual at SAH.
Appearance:
Volunteers are a representative of Saint Anthony Hospital. All volunteers must be clean, well groomed and appropriately dressed for the job. Scrubs may be provided for specific units and must be always worn. Identification Badges must be always worn while on SAH premises.
This is to acknowledge that I have received and reviewed the Code of Conduct for SAH Employees, Volunteers, and Contractors. I agree to comply with the standards contained in the code and all related policies and procedures as expected as part of my continued employment or association with SAH. I acknowledge that the code is a statement of principles for individual and business conduct and does not constitute an employment contract. I will promptly report any potential violation I become aware of to my supervisor or the compliance officer. I understand that any breach of the Code of Conduct or corporate compliance policy or procedures is grounds for disciplinary action, up to and including discharge from employment, volunteers, and contractors.
